1간호법 제정에 관한 검토

이 논문은 지금의 코로나 시국에 경험하는 가장 중요한 문제 가운데 하나인 ‘의료인력 부족’과 관련하여, 현재 국회에 계류 중인 간호법(안)에 대한 검토를 현행 의료법에 대한 비판적 시각에서 진행한 글이다. 글의 전반부에서는 입법의 연혁과 현황을 살펴보았다. 대략 45년 이상의 기간 동안 추진해 온 바 있는 간호법 독립의 실패 연혁을 간략히 살펴보고, 현재에도 변함없는 찬반론의 주요 쟁점에 대하여 검토하였다. 간호 업무의 종속성 문제, 전문간호사 제도의 실효성 문제, 간호조무사의 지위 문제 등이 그것이다. 글의 중반부에는 이렇듯 대립하는 쟁점들과 관련하여 현행 의료법과 계류 중인 간호법(안)의 내용들에 대하여 구체적인 검토를 진행하였다. 특히 현행 의료법이 가지고 있는 문제점을 간호법(안)을 통하여 얼마나 해결할 수 있는지 상세히 분석하는 것이 주요한 연구의 내용이었으나, 다소 실망스러운 결과를 얻었다. 간호사의 업무 범위와 처우 개선의 문제에 대하여는 간호법(안) 제정으로 실효성을 기대해 볼 수 있겠으나, 전문간호사나 간호조무사에 대한 정책은 ‘해결이 아닌 외면’으로 규정되어 있기 때문이다. 글의 후반부에는 이렇듯 의료 현실을 외면한 지나친 신중론에 대한 비판적 검토를 진행하였다. 의료체계에 존재하는 수직적인 구도와 여성이 절대 다수인 간호사에 대한 권력적 편견, 그리고 이권의 고수와 경제성의 논리로 일관하고 있는 현행 의료시스템상의 문제점들을 주요 쟁점을 기준으로 정리하여 비판적으로 분석하였다.


This article is a review of the Nursing Act (bill) currently pending in the National Assembly, in relation to the “shortage of medical personnel” which is one of the most important problems experienced in the current Corona situation, based on a critical perspective on the current medical law. The first part of this article is allotted to reviewing the history and current status of legislation. The history of the failure of nursing law independence, which has been promoted for more than 45 years, is briefly reviewed, and the main issues of pros and cons that remain unchanged even today are reviewed. These are the dependency problem of nursing work, the effectiveness problem of the professional nurse system, and the status problem of the nursing assistant. In the middle of the article, in relation to these conflicting issues, the contents of the current Medical Service Act and the pending Nursing Act (bill) were reviewed in detail. In particular, the main study was to analyze in detail how much the problems of the current Medical Service Act can be solved through the Nursing Act (bill), but the results were somewhat disappointing. The effectiveness of the enactment of the Nursing Act (bill) can be expected with respect to the problem of the scope of work and improving treatment of nurses, but the policy for professional nurses and nursing assistants is prescribed as ‘not to solve, but to ignore’. In the last part of the article, a critical review of the excessive prudence that ignores the medical reality was conducted. The vertical structure that exists in the medical system, the power bias toward nurses, who have an absolute majority of women, and the problems in the current medical system, which are consistent with the logic of adherence to interests and economic feasibility, are summarized and critically analyzed based on major issues.

치매노인의 증가와 더불어 간호사의 과업인 치매돌봄의 질적 향상을 위해, 휴머니튜드 돌봄을 실천한 간호사의 인식을 파악하고, 휴머니튜드 돌봄에 대한 기초자료를 제공하고자 하였다. 본 연구의 대상은 휴머니튜드가 시범적으로 실천된 I 광역시의 노인치매요양병원 간호사 3명이며, 질적 사례연구로 일대일 심층 면담을 수행하였다. 연구 결과 휴머니튜드 돌봄을 실천한 간호사들이 인식한 것은 휴머니튜드에서 제시하는 ‘자유, 평등, 우애’로 인간이 지녀야 할 기본적인 인권으로 나타났다. 본 연구는 간호사의 인식에 따른 치매돌봄 효과의 상호작용을 알 수 있었으며, 인권을 기반으로 하는 휴머니튜드 돌봄을 제시하고, 휴머니튜드 돌봄 방법을 구체적으로 제시했다는 데 의의가 있다. 사회복지정책 차원에서 휴머니튜드 돌봄의 도입과 전문인력 양성, 관련 종사자의 의무교육이 필요하며, 이는 신체적, 정신적 돌봄이 필요한 대상으로 확대되어야 한다.


In order to improve the quality of dementia care, which is a task of nurses, along with the increase of the elderly with dementia, it was intended to identify the perception of nurses who practiced Humanitude care and provide basic data. The subjects of this study were three nurses at the Elderly Dementia Nursing Hospital in I Metropolitan City, where Humanitude was demonstrated, and in-depth one-on-one interviews were conducted as a qualitative case study. As a result of the study, what nurses who practiced Humanitude care recognized was the “liberty, equality, and fraternity” presented by Humanitude, indicating basic human rights that humans should have. This study shows the interaction of dementia care effects according to the perception of nurses and is meaningful in that it presents Humanitude care based on human rights and specifically presents Humanitude care methods. In terms of social welfare policy, the introduction of Humanitude care, training of professionals, and compulsory education of related workers are required, which should be expanded to those requiring physical and mental care.

배경 : 우리나라 간호조무사 직무를 분석하여 간호조무사 자격시험 개선 사항을 의논하고자 한다. 방법 : 질적 연구 방법으로 간호조무사 직무, 특성을 이해하기 위하여 관련한 논문과 보고서를 조사하였고, 간호조무사 관련 전문가들(특성화고등학교 교사, 간호학 교수, 간호조무사 양성학원, 의사, 한의사)과 세미나 및 델파이 조사를 통하여 심도있는 간호조무사의 직무 특성 및 자격증 취득에 관하여 확인하였다. 추가로 간호조무사 자격시험 개선을 위한 포괄적 의견 수렴을 위하여 포커스 그룹 인터뷰를 10명의 간호사와 7명의 간호조무사 양성 교사 및 학원장 참여로 시행하였고, 또한 양적 연구 방법으로 256명의 간호사 및 간호조무사 대상의 온라인 설문 조사를 통해 질적 연구 결과를 확인하는 체계로 하였다. 결과 : 연구 결과 우리나라 간호조무사의 직무 범위는 상당히 포괄적이고 다양하였으나, 현행 간호조무사 자격시험이 임상에 요구되는 전문적 직무 수행을 위하여서는 한계가 있었다. 연구 결과 간호조무사 자격시험은 직무기반 개선의 필요성이 있고 이론적 학문 기반 평가보다는 “감염 예방”, “응급 대응” 등 실무 중심에 기반하여 문제 출제가 이루어질 필요가 있다는 의견이 다수였다. 결론 : 간호조무사 자격시험 개선을 위해서는 간호조무사가 실질적으로 행하여지는 분야(감염과 응급)에서의 시험 출제 강화 및 교육이 이루어져야 임상에서(1차 병원 및 환자) 높은 요구도를 만족시킬 수 있겠다.


Background : It aimed to analyze the duties of nurse assistants and to provide recommendations for improving the Korea Nurse Assistant Licensing Examination (KNALE). Methods : In order to understand the job characteristics, job content and requirements and KNALE topics were reviewed. Furthermore, the experts’ opinions concerning nurse assistants (specialized high school teachers, nursing and health professors, nursing academy directors, physicians, and oriental medicine doctors) were assessed. A seminar for convergence, a focused group interview with 10 nurses and 7 health scientists, was done. The online questionnaire for 265 nurses, nurse assistants, health science majors, and medicine majors were also conducted. Results : The survey from experts and nurse assistants revealed that the job characteristics of a nurse assistant are diverse and that the KNALE is structured in a way that does not match the professionalism required on the field. The job-based exam items should be included to improve the KNALE. Specifically, the number of practice-based items should be greater than that of theory-based items and should emphasize on “infection and injury” and “emergency response” areas. The inclusion of a former nurse assistant who can contribute to items production and item review is highly favored to improve the items’ suitability to nurse assistant’s job. Conclusion : It is necessary to improve the examination items by placing a greater emphasis on areas of “infection and injury” and “emergency response,” because a high level of such knowledge is demanded from nurse assistants who work in hospitals.

Objectives: This study determined the associated factors with low back pain in intensive care unit nurses. Methods: In this cross-sectional correlational study, 85 nurses working at five intensive care units completed questionnaires on the low back pain using visual analogue scale (VAS), use of body mechanics, work-related factors, the Oswestry disability index (%), the FACIT-fatigue, and the mini-sleep questionnaire. A back pain score of ≥4 was considered significant. Data were analyzed with χ2 test, unpaired t-test, Pearson correlation analysis and logistic regression. Results: Forty-five nurses (52.9%) had a significant low back pain score of ≥4 (VAS). Higher low back pain was associated with greater use of body mechanics (r=0.37, p=0.001), higher disability index (r=0.72, p<0.001), greater fatigue (r=0.59, p<0.001), and poorer sleep quality (r=0.36, p=0.001). Low back pain was associated with heavy workload (odds ratio, OR: 4.20, 95% confidence interval, CI: 1.40-12.58) and long standing to work (OR: 3.54, 95% CI: 1.20-10.48). Low back pain was associated with fatigue (OR: 1.18, 95% CI: 1.08-1.30) after adjusting covariates. Conclusions: Low back pain is associated with a heavy workload, long-standing to work, and greater fatigue among intensive care unit nurses.

목적: 본 연구는 코로나 감염병 대유행 상황에서 코로나전담병원에 근무하는 간호사들의 근무현황, 국가 및 지방자치단체의 지원체계 적정성 및 간호전문직 자부심을 조사하기 위하여 시도되었다. 방법: 대구에 소재한 코로나전담병원에 근무하는 첫 대유행 기간 동안 근무한 간호사 157명을 대상으로 2021년 1월 15일부터 2월 15일 까지 조사한 서술적 횡단연구이다. 설문지를 통해 근무현황, 국가 및 지방자치단체의 지원체계 적정성 및 간호전문직 자부심에 대하여 조사하였으며, 자료는 기술통계로 분석하였다. 결과: 간호사는 인력부족으로 월 7-8일, 하루 2.06시간 초과근무를 하였으며, 간호사의 97.5%가 전염에 대하여 두려움을 나타내었다. 또한 간호직무의 감염위험성에도 불구하고 국가 및 지방자치단체의 지원이 부족하다고 응답하였다. 간호전문직 자부심은 평균 3.44점으로 90% 이상의 간호사가 간호사의 전문성 향상을 인지하였다. 결론: 과중된 업무에도 일선의 간호사들이 제대로 보상받고 있지 못함을 시사하며, 이를 개선하기 위한 정책이 수립되어야 한다.


Objectives: The purpose of this study was to investigate experiences such as working status, adequacy of support system from the nation and local government, and nursing professional pride of nurses working at a Coronavirus disease-2019 (COVID-19) designated hospital during the early COVID-19 pandemic in Daegu Metropolitan City, South Korea. Methods: This study was a cross-sectional study. Participants were 157 nurses who cared for 851 patients with coronavirus infectious disease during the initial pandemic from February 18 to May 18, 2020. Work-related characteristics, national/local government/institutional support provided for risky nursing care, and nursing professional pride (Korean-Nursing Professional Pride) were surveyed from January 15 to February 15, 2020. Data were analyzed by descriptive statistics. Results: Nurses worked 21 to 22 days with 7 to 8 days of overtime working a month and 2.06 hours of overtime working a day due to a shortage of nursing staff. Of them, 97.5% of nurses demonstrated concern about the transmission of COVID-19. Despite the working conditions, many participants perceived national and local government support systems for challenging works as insufficient. However, the nursing professional pride was 3.44, and more than 90% of nurses perceived increased professional awareness of nurses. Conclusions: This study suggested that frontline nurses were overworked due to the explosive increase of patients and inadequately compensated, and policies should be established to improve these issues.

Purpose: The purpose of this study was to analyze the operation trend of the Disaster Nursing Education Course (DNEC), developed by Korea Armed Forces Nursing Academy. Methods: This study analyzed learning concept, teaching methods, class time, and achievement of participants' educational objectives in DNEC. In addition, the relevance was identified by comparing the achievement of educational objectives with International Council of Nurses (ICN) Disaster Core Competency Version 2. Results: The learning concept of DNEC was identified as five major categories of 'disaster overview, disaster preparation, disaster response, disaster recovery, and Chemical, Biological, Radiological, Nuclear and Explosive (CBRNE)', and nine teaching methods were used. Among the major categories, 'disaster response' and 'CBRNE' accounted for the most class hours. On the trend line of class hours, 'CBRNE' showed an increasing trend, and the remaining four categories showed a decreasing trend. Among the teaching methods, lecture and group discussions occupied the most time. In the trend analysis, teaching with high fidelity simulators and standardized patients showed an increasing trend. Participants' achievement level of educational objectives was 4.13 out of 5 points on average. DNEC educational objectives achievement items were consistent with 28 of the 35 of ICN disaster core competency. Conclusion: This study is meaningful in analyzing operational trends for the first time. The processed data would contribute to improving the future DNEC, and provide implications for development direction of disaster nursing education in Korea.

Purpose: This study was conducted to understand the meaning of the work performance of infection control nurses (ICNs) in general hospitals during the early COVID-19 pandemic. Methods: This was a qualitative study to explore and describe the implications of the work performance of ICNs during the early COVID-19 pandemic through a phenomenological approach. One-on-one in-depth interviews were conducted with ICNs who had been working since before the COVID-19 pandemic in the infection control department. Results: The topics brought up by the participants were categorized into five themes. The themes included “confusion caused by the early COVID-19 pandemic,” “exhaustion due to an explosion of work related to COVID-19,” “active response as an ICN,” “emphasis on the importance of infection control,” and “overcoming the early COVID-19 pandemic”. Conclusion: It is necessary to strengthen the competence of ICNs as experts and establish a career management system to overcome the confusion and physical and psychological difficulties experienced by ICNs.

Purpose: This study was investigated the effects of job stress, communication type, and coaching competency on job commitment. Methods: The subjects of this study were 153 preceptor nurses at a general hospital located in S city. The data collection period was from April 1 to April 15, 2021. A self-report questionnaire was used as the data collection method. Results: (β=-.29, p<.001), ptimism coaching competency (β=.30, p<.001), dysfunctional communication type (β=.16, p=.020). Conclusion: For job immersion, job stress management is required, and in particular, in relation to job tension, it is necessary to prepare a plan to increase job immersion through management because nurses perform the role of educating new nurses as well as their duties as nurses optimism plays a very important role in coaching competency. Therefore, it is necessary to increase professionalism developa systematic preceptorship program.

Purpose: This descriptive research was conducted to identify the effects of work complexity, role conflict, knowledge of traditional Korean (Hanbang) nursing, and grit on the job satisfaction among traditional Korean nurses. Methods: The survey was conducted from September 7, 2020 to February 10, 2021 on 155 Hanbang nurses. Data were analyzed by t-test, one way ANOVA, Pearson’s correlation coefficient, and multiple linear regression with the SPSS statistics 26.0 program. Result: The factors affecting job satisfaction of Hanbang nurses were work complexity, knowledge of Hanbang nursing, age, and experience in traditional Korean medicine hospitals. Conclusions: It is necessary to promote Hanbang nursing related education in regular and clinical area in order to improve the job satisfaction of Hanbang nurses. In addition, an organizational effort is required to increase long-term job satisfaction.

Purpose: This study investigated whether parenting stress, work-family conflict, resilience affect retention intent in Korean nurses returning to work after parental leave. Methods: The participants in this study were 111 nurses recruited from 10 hospitals in Korea, who were working after formal parental leave from their hospitals. Collected data were analyzed using descriptive statistics, the t-test, analysis of variance, the Mann-Whitney U-test, Pearson correlation coefficients, and hierarchical regression. Results: Retention intention (33.80±7.78), parenting stress (101.70±17.57), and resilience (85.02±12.75) were at greater than moderate levels and a midpoint level of work-family conflict (29.63±7.00) was noted in this sample of mostly women nurses in their 30s. The factors affecting retention intent were parental leave duration, the number of times that participants had taken parental leave, health condition, work-family conflict, and resilience. The total explanatory power of these variables was 36.7%. Retention intent had a negative correlation with parenting stress and work-family conflict. Conversely, retention intent was positively correlated with resilience. Conclusion: This study supports the need for flexible adjustment of returning nurses’ working hours and family-friendly policies to promote balance between work and family. It is also necessary to develop and apply measures that boost resilience and support health improvement for nurses returning to work.As nurses are often assigned to new areas of work upon return, training programs to aid their adjustment may also be helpful.
